Attention US Airways “West” employees, who live in the
metropolitan Phoenix area:
Mail order prescription drug services for West
employees (covered on West medical benefit plans) are
handled out of Caremark’s state of the art processing
facility south of the airport in Phoenix. You can
mail in or drop off your mail order and have it
conveniently shipped to your home address shortly
thereafter. However, if you travel a lot and don’t
have anyone at home to clear your mail box, you may be
concerned about the summer heat in Arizona. Please be
assured that Caremark takes extra precautions and has
special packaging and shipping procedures to ensure
the safe delivery of your products, in accordance with
both FDA guidelines and drug manufacturers’
directions. In addition to these special procedures
that are in place all the time, Caremark now has a
new service (exclusive to the Phoenix Mail Order
facility only) where you may DROP OFF AND PICK UP your
mail order prescriptions at that facility. Your mail
order will be kept secured in a climate controlled
environment until you come to get it. This added pick
up service is available from May 1st to
October 1st.
Here’s the procedure:

Completely fill out a new mail order form, including
the following information:

Employee and dependent (if applicable) information
(i.e. employee Caremark ID number, home/cell
phone, etc.).

Payment information.
-
Indicate Shipping request on mail order form –
print “CUSTOMER PICK-UP” in address area.

Include new prescriptions &/ or refills.
-
Complete separate Plan Participant Authorization
form if picking up for other adult family members
(privacy rules require you to do this).

Drop off your order (new or refill) via the Caremark
receptionist at the Phoenix Mail Order facility
between 8:00 a.m. – 5 p.m. MST, Monday – Friday
(except holidays) at 4121 E. Cotton Center Blvd.
Phoenix, AZ 85040.

The first time you use this process you must drop
off any new or refill orders at the Caremark
receptionist at the actual Phoenix Mail Order
facility – do not use the regular drop box at the
building next door where all your mail orders that
go to your home address can be dropped.

Subsequent summer refill requests for this pick-up
service can be phoned into an exclusive toll-free
number at 1-866-579-7040.

Allow up to 5 business days for your order to be
ready for pickup.

Once the order is completed, a Participant Services
team member will contact you by phone to arrange
your pick up. The Caremark specialists who handle
the pick-up service are: Teresa Christoff, Ann-Marie
Hulan and Tim Pfaff
Please refer any questions to the above mentioned
phone number and specialists.
*** REMEMBER, YOU MUST USE THIS PROCESS EXACTLY IF
YOU WANT CUSTOMER PICK UP MAIL ORDER SERVICES. You
cannot use any of the regular mail order processes
like the Caremark website, Customer Care or IVR,
regular mail in or drop-off box at the next building,
to select Customer Pick up services***
