Section 33 - Flight PAC ContributionsA. The Company shall provide a means for payroll deductions for any Flight Attendant who completes a Flight PAC Wage Deduction Authorization Card, as provided in Paragraph D., below. B. The Union shall bear the administrative costs, if any, incurred by the Company in processing and forwarding contributions to Flight PAC to the Union. C. Flight PAC contributions shall be deducted each month from the paycheck that contains pay exceptions and shall be remitted to the Union as soon as practical thereafter, but in no event later than thirty (30) days after the date of the deduction. D. Flight PAC Wage Deduction Authorization Card
and represented by the Association of Flight Attendants, hereby authorize and direct America West Airlines, Inc. to deduct monthly from my wages the sum of $______.00 and forward this amount monthly to the AFA Flight PAC, 1275 K Street N.W., Fifth Floor, Washington, DC 20005. I hereby authorize the above amount to be deducted each month from my paycheck that contains pay exceptions each month. I have executed this wage deduction authorization card voluntarily without any coercion, duress, or intimidation, and none of the monies deducted are a part of my dues or membership fees to the local union. I understand that I may refuse to contribute without reprisal. This authorization and the making of payments to the AFA Political Contribution Committee, known as the AFA Flight PAC, are not conditions for membership in the Union or of employment with the Company. I further understand that the money will be used by the AFA Flight PAC to make contributions and expenditures in connection with federal elections. I further certify that I am either a United States citizen or a foreign national lawfully admitted to the United States for permanent residence as defined by section 101(a)(20) of the Immigration and Nationality Action (8 O.K.. 1101(a)(20)). This authorization card shall remain in full force and effect until revoked by me, pursuant to the provisions of the Agreement between my airline and the Association of Flight Attendants.

E. In the event the Company, including any subsidiaries, branches, divisions, or affiliates, no longer offers a payroll deduction plan, check-off system, or other plan which deducts contributions from the payroll or dividend checks of stockholders or executive or administrative personnel, the Company shall have the right to revoke authorization for payroll deduction under this section provided the Company gives the Union at least ninety (90) days’ prior written notice. In the event the Company subsequently intends to re-establish a Company deduction plan as described above it will so notify the Union prior to the re-establishment, and the Union prior to re-establishment and the Union Flight PAC Wage Deduction shall be reinstated at the same time.
